I accept that by cancelling the Sentricon or Termite Renewal I release Go-Forth of all liability of wood destroying organisms and that should I suffer any damage from wood destroying organisms, Go-Forth will take no responsibility.

I also acknowledge the substantial investment made in getting the property under termite service agreement and I understand that by cancelling my termite service a large fee may be required to reinstate service.

Sentricon products must be removed after cancellation.  We will be stopping by to remove the Sentricon bait from the stations and caps at our earliest possible convenience. *

I have selected to cancel my Christmas Light Service.  If lights are still up Go-Forth will immediately come remove the lights.  If you are attempting to cancel prior to Christmas but after installation please wait to cancel until after Christmas then cancel that way you may keep your lights up this season until after Christmas. *

Reminder: If you are still under the minimum service agreement period per your agreement, there will be a charge to cancel my account early (maximum charge "$50 buyout" per service type). We will try not to bother you. We will check your service agreement for you. If there are multiple services and some are under the minimum service agreement period and some are not, we will only charge the $50 buyout for each service that is still under the minimum service agreement period.
